Health and Homelessness Projects

There are currently two Health and Homeless Projects coordinated by St Mungo's. These projects include the Health and Homeless Project (HHP) in Hammersmith and Fulham (H&F) and the Homeless Health Coordination Project (HHCP) in Westminster.  Both projects work to improve health outcomes with clients living in Supporting People funded accommodation in H&F and Westminster, and are managed by coordinators who liaise between housing and health services across the borough.

Activities delivered include:

  • Coordinating staff and client training
  • Organising health screening events (e.g. TB, Hep C, Dental)
  • Managing the annual client conference and Wellbeing Fair
  • Producing monthly newsletters
  • Chairing bimonthly Health Action Group meetings
  • Collecting quarterly statistics from housing providers
  • Ensuring clients are appropriately referred to health services
  • Developing joint working relationships

This health audit revealed the high health needs of clients in Westminster, and makes recommendations on how these could be improved.
