The Hospital to Home Project in Hammersmith and Fulham was funded by the Department of Health. The service was delivered in partnership with Groundswell between November 2013 and June 2014 in Charing Cross and Hammersmith hospitals. A project coordinator and outreach worker offered homeless patients support to access housing and health services in their local area.
The King's Health Partners (KHP) Pathway Homeless Team is a multi-professional team of homeless specialists including two GPs, general and psychiatric nurses, housing liaison workers, an occupational therapist and a social worker. The team currently works across the Guy's, St Thomas' and King's College Hospital sites. They provide support and advice that is often needed to improve the quality of health and social care for homeless people attending these hospitals. They have also provided training and a resource booklet for hospital staff to support and signpost homeless patients to appropriate services.
The KHP Pathway service started in January 2014 and is working in partnership with St Mungo's, St Giles Trust and The Passage.
